each (includes tax and shipping) (1 for $15, 2 for $30, 3 for $45, etc.) |

- Fill out the
Address, Quantity, Price fields (E-mail/Phone are optional but helpful) |

- Make out
personal checks, cashiers checks, money orders to "Gary Michael Bass" |
| |
- If paying by
credit card, fill out the fields for CardNumber/Expiration |
| |

- Accepted cards
are VISA, MasterCard, American Express and Discover |

- Shipping
address will be the same as the billing address |
